Intake Information

Flat Rock Care Center accepts children ages 6 years to 18 years. While residents may stay in our care until they are 21, we do not admit persons over 18 years old.

Most residents come to us through referral from the County Board of MR/DD of their home county. Case managers and/or Service Support Administration staff should contact the Program Associate, Gwen Ladd at 419-483-7330 (ext. 1109).

The intake process includes application materials, intake meetings with the perspective resident, parents/guardian(s) and Case Management and FR Interdisciplinary Team (IDS), an on-campus pre-admission visit of 5-7 days, and a formal recommendation of the IDS team.

Flat Rock Care Center currently has a waiting list for males and females.

The Facility agrees to provide services to persons whose needs can be met by the Facility, without regard to disability, race, color, religion, national origin or ancestry, sex or age, including, but not limited to, making reasonable accommodations to the individual's physical, mental or behavior disabilities to the extent required by law, unless the Facility demonstrates that the accommodation would impose an undue hardship on the operation of the program.
